Charleston's Hope Integrative Medicine Launches New Care Coordination Program Built on Its Integrated Model

Charleston, United States, November 8, 2026 -- FOR IMMEDIATE RELEASE

CHARLESTON, S.C., Nov. 6, 2026 — Hope Integrative Medicine, an evidence-based allopathic adult medical practice in the West Ashley area of Charleston, today announced the launch of its care coordination program, which includes Chronic Care Management and Behavioral Health Integration, extending its integrated care model beyond the exam room and into the weeks between appointments.

Chronic conditions do not fit neatly into scheduled appointments. High blood pressure, diabetes, depression, anxiety and recovery from addiction are lived with every day, while most outpatient care is still built around the visit. The program pairs eligible patients who elect to enroll with a dedicated care manager who works throughout the month with the patient's provider and the specialists involved in their care: following up on symptoms and results, keeping medication lists and care plans current, and giving patients streamlined access between visits to someone who already knows their care and where it currently stands.

The program builds on what sets the practice apart. At Hope Integrative Medicine, primary care, mental and behavioral health care, and addiction medicine and recovery care are delivered by one clinical team working from one chart. Some patients come only for primary care, some for mental health care or recovery care, and others receive care from all three. A patient managing blood pressure, depression and recovery at the same time does not need three practices, three intake processes and three separate plans. The care manager, the primary care provider and the behavioral health clinicians already work side by side, and the care manager also coordinates with the patient's outside specialists and other providers on the patient's behalf. Patients are no longer left on their own trying to keep every provider informed and on the same page.

“Chronic conditions don't keep office hours,” said a company spokesperson. “Our patients live with them every day, and their care should reflect that. Coordinating care between visits, inside a practice where medical care, mental health care and recovery care already share one team and one chart, is what truly integrated care should look like.”

The approach rests on a substantial body of evidence. In the IMPACT trial, published in JAMA in 2002, 45 percent of older adults with depression who received collaborative care, the model behind Behavioral Health Integration, had at least a 50 percent reduction in symptoms after 12 months, compared with 19 percent in usual care. A Cochrane review of collaborative care found it is associated with significant improvement in depression and anxiety outcomes compared with usual care. An independent evaluation of Medicare's Chronic Care Management services, commissioned by the Centers for Medicare and Medicaid Services, reported slower growth in hospitalizations, emergency department visits and Medicare spending among participating patients compared with similar patients.

Medicare, some commercial plans and some Marketplace plans cover these services for eligible patients who elect to enroll, and coverage and coinsurance vary by plan. Current patients can ask about enrolling at their next visit, by message or by phone. People who are new to the practice can request an appointment at hopeintegrativemed.com.

Japan man with ‘grudge against society’ stabs bookshop worker

Japanese police arrested a 33-year-old man who admitted to stabbing a bookshop worker in Osaka on Sunday, a spokesman said, adding that the suspect said he “held a grudge against society”. The man, identified as Satoshi Okamoto, allegedly used a kitchen knife to stab the worker – who called police immediately afterwards – at a branch of national chain Maruzen & Junkudo in the heart of Japan’s second largest city. Police did not elaborate on the victim’s condition, but there was no fear for his...

AI will soon master biology, unleashing new safety risks, Tencent AI chief warns

Artificial intelligence models will soon be powerful enough to master biology, presenting both benefits and serious risks, Tencent Holdings’ chief AI scientist and former OpenAI star researcher Vinces Yao Shunyu said during a rare public appearance on Saturday. The assessment comes as frontier AI systems are increasingly being applied to medicine and biology, which could accelerate discovery of new drugs and other treatments. However, experts also warn that malicious actors could use AI to...

China’s Pudu Robotics, which makes 80% of its sales abroad, takes on Tesla in humanoids

Humanoid robots represent the next frontier for Pudu Robotics, according to founder and CEO Felix Zhang Tao, as the global commercial robotics leader takes on Elon Musk’s Tesla in the growing sector and explores manufacturing in Europe, its largest overseas market. “It’s definitely a competitor, but a long-term one,” Zhang told the South China Morning Post in a recent interview, adding that Tesla was a “very good” rival whose high profile had driven innovation and broadened market awareness in...

India questions US ‘double standards’ as Pakistan secures F-16 upgrade deal

Washington’s decision to upgrade Pakistan’s F-16 fighter jets has drawn accusations of double standards from critics in India, who question why the United States is strengthening a nuclear-armed ally while waging war against Iran over its nuclear programme. Although the US$686 million military modernisation package does not include nuclear weapons, analysts say that improvements to the fighter jets’ navigation, communications and survivability could boost Pakistan’s existing nuclear strike...

Why raising prices in Japan amid ‘insanely high’ inflation backfires on firms

When Japan’s “symbol of affordability” Uniqlo decided to increase the price of its basic socks last month, it sparked a backlash before the clothing titan made an immediate U-turn. That came after Italian-style family restaurant chain Saizeriya issued an almost grovelling apology as it decided to add 20 yen (US$0.13) to the price of its trademark Milanese Doria, lamenting its “lack of effort”. The very public angst, including in one of the world’s most recognisable clothing brands, highlights...

Southeast Asia’s green push brings solar power, but who pays to keep it on?

For most of his life, Osman Uki lived in a world dictated by the sun, relying on kerosene and leftover cooking oil to light his home in a remote indigenous village in Malaysia. That changed in April when solar power finally reached Kampung Bertang in the state of Pahang, allowing families to power electric lights, refrigerators and rice cookers. For Osman, one of the biggest changes is what happens after sunset. “We are happy with the light we have at night. The kids can read at night now. We...

Indonesians furious as Batam data centres drain taps: ‘we cannot cook without water’

Cooking and bathing became difficult when taps ran dry in Batam, an Indonesian island where residents are protesting against data centres springing up to power the global artificial intelligence boom. The area had suffered a “long drought” caused by low rainfall, the local water utility operator said, as a strong El Nino cycle exacerbates dry weather. But, as in other countries, anger has been directed at the data centres, warehouse-like facilities that keep apps and AI tools running, with water...

Japan rethinks its stalking laws after Pokemon store murder

Long before Moe Harukawa was killed, she lived in fear. The 21-year-old shop assistant had reported her ex-boyfriend to police and a court had ordered him to stay away from her. Yet weeks later, Taiki Hirokawa, 26, walked into Harukawa’s workplace – a packed Pokemon store in Tokyo’s Ikebukuro district – and stabbed her repeatedly in the neck and torso before turning the knife he was carrying on himself. Both later died in hospital. Harukawa’s gruesome murder in March at the hands of a man who...

When her prescription for Mounjaro became too expensive, Bey* did what a growing number of Malaysians are doing: she found a cheaper vial online. The 34-year-old from Kuala Lumpur started on the weight-loss drug about a year ago and hasn’t looked back since. “I didn’t feel like eating, I didn’t feel hungry,” she said, recalling flying to Jakarta for a “food trip” recently during which she barely ate. Reviews on Threads and TikTok led her to online sellers offering “tirzepatide”, Mounjaro’s...
